No, but if I do I will upload it! I'm honestly not sure where it is at this point; I'd need to scour my parents' house to find it I think; I lost track of some of my stuff since I'd moved out. But if I do I will scan and upload it! No need to keep asking, since I'm not sure when it might be that I find the thing. But rest assured that if I find it I'll upload it -- just don't know when that will be :(
